Welcome to our Online Patient Center! Here you can get information about what to expect during your visit with our doctors as well as important forms you can complete before your appointment to save time.

We want your visit to be as stress-free as possible. Our staff will do everything we can to make sure your experience is pleasant and convenient. If you have any questions about these forms, please don’t hesitate to contact us.

What to Expect

When you visit our office, you will be warmly welcomed and we can answer any questions you may have about completing the paperwork so that Advanced Family Eye Care may see you. Our team will work together to evaluate your eyes and provide relevant eye care treatment information, review your vision benefits and payment options, fit you for glasses or contact lenses, and focus on exceeding all of your expectations. If at any time you have questions about the process or your specific health information, please ask.

How To Access Your Patient Portal

Step 1: You should have received a welcome email for your personal portal with our office. If you do not have a log in, please text us to reset at 704-822-9920. Please use the temporary password in that email to log in and set up your personal password. (Do NOT copy and paste this password, you will need to manually enter it in order to log in.)

Step 2: Once you have set up your patient portal, please access the portal.

  • Please click on our Patient Portal Link on our home page or on our Patient Center Page
  • Our portal allows you to access information about your visits and send us information securely.

Step 3: Once on the Patient Portal website, please log in.

Step 4: On the Portal Face Page: after signing in you will see 3 main tabs.

  • Practice Forms: is where the HIPAA, Financial & COVID-19 signature page is located.
    • Click on each document to open
    • Follow the steps to Sign each document digitally

Step 5: Please ensure that the following three items have been completed prior to your visit:

  1. HIPPA and Financial Documents accessible through Patient Portal
  2. COVID-19 Questionnaire accessible through Patient Portal
  3. Text photos of your insurance cards to our office at 704-822-9920

Advanced Family Eye Care believes in providing the best value for you and your family. We have products and pricing that aims to accommodate all budgets and our office participates in most insurance programs. We offer discounts for patients without insurance. We offer Express Bundling for EyeWear at a budget friendly price. Ask our Optical Department about all our products.

Our office accepts cash, check, and major debit and credit cards including MasterCard and Visa, as well as Care Credit.
